We believe in meeting people where they are—physically, emotionally, and personally—and ensuring every interaction reflects dignity, comfort, and connection. Position Overview We are seeking a warm, professional, and highly relational Community Liaison / Comfort Concierge to serve as a key connection between Caring Hearts Hospice, patients, families, and community partners. This role focuses on outreach, relationship-building, education, and supportive engagement in the community setting. It is not a clinical role, but requires strong experience navigating healthcare environments and communicating effectively with referral partners and care settings. Key Responsibilities Serve as a welcoming and supportive point of contact for patients, families, and community partners Build and maintain strong referral relationships with hospitals, facilities, Adult Foster Homes, and community organizations Conduct consistent community outreach, visits, and relationship development throughout the service area Represent Caring Hearts Hospice at facilities, networking opportunities, and community events Provide non-clinical comfort support, education, and resource guidance to patients and families Assist with smooth onboarding and orientation of new patients in a compassionate and reassuring manner Collaborate closely with clinical and administrative teams to ensure timely communication and continuity of care Support family and referral partner understanding of hospice services, eligibility, and resources Maintain accurate documentation of outreach, visits, and referral activity in CRM systems Qualifications Minimum 1+ year of experience in healthcare required (hospice, home health, senior care, medical office, or related setting) Experience in hospice, palliative care, senior services, or healthcare marketing strongly preferred Background in Adult Foster Homes, facility relations, or community outreach highly desirable Strong interpersonal and relationship-building skills with referral partners Exceptional communication skills (verbal and written) Professional, calm, and emotionally intelligent presence in sensitive situations Ability to work independently in a field-based role with frequent travel Strong organization, follow-through, and time management skills Valid driver's license and reliable transportation required Preferred Attributes Experience working within the hospice or senior care continuum Natural relationship builder who is comfortable in healthcare facilities and community settings Ability to balance compassion with professionalism in end-of-life care conversations Strong networking skills and ability to maintain long-term referral relationships What We Offer Meaningful, mission-driven work supporting patients and families Supportive and collaborative team environment Competitive compensation and benefits Mileage reimbursement Opportunities for growth within a growing hospice organization
