Job Description

The Mifflin-Juniata Area Agency on Aging affiliate of Mifflin-Juniata Regional Services is hiring a full time Community Case Aide. Some of the job duties include: Perform intake and referral activities. Make appropriate notations in consumer records and input required information into assigned computer database. Maintain client records according to established procedures. Complete routine service delivery authorizations for Options personal care. Complete periodic audit for quality assurance of programs. Assist Care managers with maintenance of monthly activities and referrals. Review program service invoices monthly for accuracy. Provide FCSP benefits counseling, as assigned. Facilitate the initiation of FCSP services for new caregivers by assisting with form completion and submission, arranging for needed services and obtaining supplies, and educating consumers on program procedures and parameters. Review and reconcile submitted FCSP reimbursement requests for accuracy and forward to the Fiscal Department. Maintain an accurate and updated list of area support groups for FCSP Caregivers. Manage the Fuel Assistance Program. Complete desk reviews in PIA. Maintain a caseload of home delivered meals consumers. Perform home visits and monitoring calls as well as completing the NAT-E assessments for those visits. Update provider services listings to ensure the information is accurate and that the consumer has an updated listing. Assist with information and referral as needed. Maintain a high level of confidentiality at all times. Attend staff meetings and related training courses as required. Perform other duties as assigned. The ideal candidate will have an Associate's degree in human services or a related field and 1 year of experience in a health or human services field. A candidate with a high school diploma or equivalent with 2 years' experience in a public or private human service agency or business, involving direct customer service experience would be considered. Must possess excellent communication and listening skills, computer skills, and have a thorough knowledge of local human service resources. Child Abuse and PA Criminal Background clearances are required.
