In-Person $20.32 - $20.32 per Hour 1 Years Experience High school graduate or equivalency certificate (GED) No Commission 40.00 hours per week / Day Shift / Full-Time Description Position Summary The Service Navigator supports individuals and families in achieving self-sufficiency through strength-based case management. This role completes assessments, helps customers identify goals, connects them with community resources, and provides ongoing support to promote stability and independence. Key Responsibilities Assess needs and develop individualized service plans with customers. Identify barriers to self-sufficiency and create action plans. Provide referrals to STEP programs and local community resources. Conduct regular follow-ups through home visits, phone calls, virtual meetings, and office appointments. Maintain accurate case notes and program documentation. Build and maintain strong working relationships with community partners. Use computer systems and Microsoft Office to support service delivery. Qualifications High school diploma or GED required; bachelor's degree in human services preferred. Minimum one year of casework or related experience. Knowledge of community resources, social service systems, and eligibility requirements. Strong communication, organization, and problem-solving skills. Ability to manage a fast-paced caseload and work independently. Valid driver's license, reliable transportation, and ability to meet physical job requirements. Ability to work respectfully with individuals from diverse backgrounds. Required clearances: PA Criminal, Child Abuse, FBI, NSOR, MVR. Additional Requirements Proficiency with Microsoft Office Suite. Flexibility for non-traditional or adjusted schedules. Participation in required training.
