The Chronic Complex Social Services Specialist responsibilities include: Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community. The Chronic Complex Social Service Specialist provides therapeutic interventions through assessment, coordination, referral, and interdisciplinary planning. They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs. Possess a clear understanding of discharge and transition planning and linkage with community resources. Demonstrates skills in planning, organizing, and managing multiple functions and complex processes. This position helps facilitate and expedite discharging of our patients with complicated and/or complex discharge needs. •This position is working specifically with patients in a Cardiac Program• Position details:

Required Master's Degree of Social Work or Related Field Experience:
Required 2 years Social Work in an acute care/hospital setting Preferred experience in Social Work in Intensive Care, Cardiac Intensive Care or Transplant unit Strongly preferred experience in Social Work with Ventricular Assistive device or Mechanical Circulatory Support device recipients
