Hospice & Community Care has been a cornerstone of compassionate end-of-life care in our community for nearly four decades. We are seeking a Hospice Community Liaison/Business Developer to work Full Time to serve our community. Our organization is fully dedicated to meeting the needs of patients and families, and helping them receive the care they need.
What You'll Do:
Build and maintain strong referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other partners to increase awareness and appropriate hospice referrals. Educate patients and families about hospice philosophy, services, and eligibility, guiding them with compassion through the referral and admission process. Collaborate closely with clinical and admissions teams to ensure smooth transitions, accurate documentation, and timely communication. Travel throughout your assigned territory to conduct outreach visits, presentations, and community events. A valid driver's license and reliable transportation are required.
What You'll Need:
Bachelor's degree in Marketing, Business, or a health-related science (e.g. Nursing, Pharmacy or Social Work) preferred. Previous experience in healthcare marketing, hospice, palliative care, or home health. Valid Driver's License and reliable transportation required. General understanding of hospice referral partners (hospitals, physician, skilled nursing facilities, and assisted living facilities). General knowledge base of the healthcare providers in our service areas: York, Chester, and Lancaster counties. Strong communication and interpersonal skills Key Benefits of Joining our

Are you comfortable traveling regularly throughout this area to visit referral partners, facilities, and families? This role requires frequent driving. Are you able to work full-time, Monday through Friday 8:00am-4:30pm, with occasional community events or educational presentations outside regular hours?
