Position Overview We are seeking a dynamic and experienced Community Liaison to support growth and strengthen referral relationships within the North East Houston and surrounding territories. This role is ideal for a relationship-driven professional with deep knowledge of home health and hospice services who thrives in a fast-paced, mission-focused environment. As a Community Liaison, you will serve as a key ambassador—educating referral sources, supporting smooth patient transitions, and ensuring our services are accessible to those who need them most. Key Responsibilities Develop and maintain strong relationships with physicians, hospitals, case managers, and community partners Promote home health and hospice programs while upholding the highest standards of quality care Assess potential patients for eligibility and coordinate care transitions Serve as a liaison between referral sources, clinical teams, patients, and families Identify growth opportunities and contribute to strategic outreach initiatives Manage territory coverage including Humble, Kingwood, Atascocita, New Caney, Etc. Ensure compliance with all regulatory and ethical standards while maximizing resources effectively

Qualifications Experience:

Required background in the home health and/or hospice industry

Education:

Graduation from an accredited college or university preferred
