Job Description

Heritage Home Health and Hospice is seeking a motivated and relationship-driven Sales Representative to help grow our Home health and Hospice services within the community of Rockwall, Greenville, Farmersville, Princeton and surrounding areas. The position is responsible for building strong referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities and other healthcare partners while helping patients and families access quality care.
Essential Functions:
Develop and maintain referral relationships within assigned territory. Grow Home Health and Hospice census through strategic account management. Conduct daily sales calls and maintain consistent weekly routes. Partner with hospitals, case managers, discharge planners, physicians, and facility staff. Educate referral sources on Home Health and Hospice services. Track referrals, pending admissions, and account activity. Attend community events and networking opportunities. Collaborate closely with clinical and operational teams to ensure excellent patient care.
Qualifications:
Healthcare sales experience preferred (Home Health, Hospice, SNF, or related healthcare field) Strong relationship building and communication skills. Self-motivated with ability to work independently. Knowledge of referral development and territory management preferred. Compassion for patients and families.
