Job Description

Cascades at Riverwalk - Midvale, Utah Full-Time | Monday - Friday Cascades at Riverwalk is a skilled nursing and rehabilitation community where compassionate care and professional growth go hand in hand. What you'll do: Advocate for resident needs and rights Maintains contact with family, in consultation with the resident, to report on changes in health, goals, discharge planning Builds relationships between residents and staff and teaches staff how to understand and support resident's individual needs Assists in resolution of grievances as voiced by resident, family, responsible party, or ombudsman Assists with documenting social service problems with appropriate approaches and time measurable goals What we offer: Competitive hourly paybased on experience 401(k) with company match Medical, dental, vision, and life insurance options Generouspaidtime off(PTO) What we're looking for: Bachelor's degree in Social Work or Human Services required; Master's degree preferred LCSW strongly preferred Minimum of one year of supervised social work experience in a healthcare setting 1 year experience in Long Term Care Experience with electronic medical records required Strong communication and organizational skills Commitment to supporting the senior population Why us: Our Social Services team care for a geriatric population that depends on consistent, knowledgeable, and compassionate care.
