• Key Responsibilities:

• + Conduct resident assessments and develop individualized care plans + Provide case management and connect residents with community resources + Support residents and families with counseling, advocacy, and service coordination + Monitor Medicare eligibility and provide required notifications + Maintain accurate documentation in compliance with federal and state regulations + Collaborate with interdisciplinary teams and outside agencies + Promote resident rights, safety, and overall quality of life •

Qualifications:

• + Bachelor's Degree in Social Work or related field (gerontology, psychology, recreation therapy, etc.) + 1-2 years of experience (geriatric setting preferred) + Strong knowledge of community resources and supportive services + Excellent communication, documentation, and organizational skills + Social Services certification preferred
