Treatment & Resource Navigator Department of Health and Human Services Hourly Range:

Job Summary:
The Treatment Resource Navigator will focus on providing full-time street outreach services to individuals and families experiencing homelessness in Milwaukee County that need medically assisted treatment and harm reduction services. This position will also work to secure indoor placements for consumers experiencing homelessness to include street outreach services, case management, and referring to local supportive housing programs.
Job Duties:
Street Outreach:
Locate and engage with individuals and families experiencing unsheltered homelessness. Collaborate with outreach teams to develop immediate housing plans and refer to housing programs.
Coordinate Care:
Develop and strengthen relationships to connect participants to harm reduction and medically assisted treatment services. Benefits acquisition. Establish community partnerships with agencies to assist individuals/families connect to harm reduction services. Additional units. Work with existing and potential landlords to find additional units for the program. Perform other duties as assigned.
Minimum Requirements:
Current resident of the State of Wisconsin. Current Wisconsin Driver's License maintained throughout employment. Must possess a bachelor's degree or higher in Social Work, Human Services, or closely related field. At least one (1) year of experience working with the homelessness population, supportive housing, and/or human services, including substance abuse and harm reduction. Intermediate knowledge of all computer and software applications such as Microsoft Word and Microsoft Excel. Must be able to travel extensively, up to 70% of work time, within the community.
Knowledge, Skills, and Abilities:
Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; knowledge of case management; knowledge of violence reduction efforts; ability to work with individuals with a wide range of needs.
