Title:
IOP Counselor Function :
The
CORE IOP
Counselor Acts as case manager/counselor for assigned clients on an intensive outpatient level.
Supervisor :
CORE Director of Clinical Services and/or designee.
General Work Schedule :
Clinical services IOP schedule is Monday, Tuesday, Thursday and Friday evenings 5:00 p.m. - 8:00 p.m. Screening, intake, individual/family sessions, case management and discharge planning will be outside the above schedule. A minimum of 40 hours a week, with on-call as directed by supervisor.
Qualifications :
1. Master's degree in counseling or related field of study. 2.
LAC / LPC / LCSW
status current or in process of completion. 3. CCGC, ICGC or equivalent in active status or pursuing. 4. Pre-Employment negative drug screen and background check. 5. Free from TB and verified by annual skin test by CORE.
Duties and Responsibilities :
1. Conducts intensive outpatient process groups, lectures, and individual sessions with clients. 2. As needed, conducts process groups, individual sessions, and family counseling in a residential treatment setting, along with providing group lectures, case management, client assessments, and chart documentation of client's treatment progress. 3. Prepares educational, group and individual progress notes for each client within 24 hours and 48 if PT (if needed). 4. Prepares and completes discharge summaries of assigned clients within 72 hours. 5. Works with assigned clients to formulate a Health & Recovery Plan and coordinate continued support services such as a counselor, GA meetings, sponsor, etc. 6. Coordinates care with Intake Specialist, fills out necessary paperwork for new clients. 7. Performs client assessments to determine the needs and severity of gambling addiction for each potential client if the Intake Specialist is unavailable. 8. Meets individually with each assigned client to go over treatment plan and objectives. 9. Completes the assessment on assigned clients within 72 hours of admission. 10. Coordinates client information on a weekly basis with the rest of the clinical staff. 11. As needed, coordinates with the medical doctor and contract RN on patient care issues. 12. If necessary, assists in supervision of intern counselors by co-facilitating/shadowing in group, assigning client tasks such as initial assessments, etc. 13. Provides outreach lectures when requested by the Director of Clinical Services. 14. Maintains assigned patient records and ensures they meet clinical guidelines. 15. Supervises the updating and creation of educational, step, and relapse prevention materials for use in treatment. 16. Attends staff and clinical meetings. 17. Other duties as assigned by Director of Clinical Services and/or designee.
Clinical Duties and Responsibilities :
1. Within scope of regulatory body credentialing, provides clinical service supporting daily programming of residential, intensive outpatient, and outpatient services of LACG. 2. Maintains clinical documentation of services provided. 3. Supports and fosters a therapeutic environment through action and supervision of other team members.
