Job Description

Sunrise Centre is looking for a Care Manager/Substance Use Disorder Counselor to join our team providing individual and group counseling services to residents of our 41 bed treatment facility serving individuals with substance use disorders and co-occurring mental health disorders. Job description as follows:
Purpose of Position:
Counsel clients with substance use disorders. Complete intake assessments, complete person-centered treatment plans, conduct counseling sessions, utilize evidence based interventions to support clients in working towards sustainable recovery. Complete diagnoses, within scope of license and degree, and support treatment of co-occurring mental health conditions.
Qualifications:
We will consider applicants with a Bachelor's Degree in Human Services related field depending on experience as we will support the training of the right candidate for Care Manager role. CADC preferred or ability to get on development plan. Would prefer person licensed to provide mental health services in the state of Michigan. Master's Degree in the Human Service field with state license as an LMSW, LLP, or LPC (limited license accepted and individuals nearing completion of Master's degree program will be considered) preferred. CADC or CAADC certification with the Michigan Certification Board for Addiction Professionals (MCBAP) or willing to be on a development plan working towards that certification required.
Primary Job Responsibilities:
Evaluate psychological, social and physiological signs and symptoms of alcohol or other drug use and abuse; determine client's appropriateness and eligibility for admission or referral Complete requirements for program eligibility and appropriateness (ASAM) Gather relevant history from the client including but not limited to alcohol and other drug use disorder using appropriate interview techniques Identify methods and procedures for obtaining collaborative information from significant secondary sources regarding clients' alcohol and other drug use and psychosocial history Implement appropriate assessment tools and techniques to facilitate understanding with clients Develop a diagnostic evaluation of clients' substance use disorder and any coexisting conditions based on the results of all assessments Provide an integrated approach to treatment planning based on the clients' strengths, weaknesses and identified problems and needs Collaborate with treatment team members or external professionals to establish initial treatment plan, including long and short term goals, and comprehensive, quality care for clients Provide relevant education to clients' using a formal didactic format with reading materials, film, formal and informal individual processes Identify client needs Sunrise Centre cannot meet, establish support systems & community resources Adhere to all applicable laws, regulations and agency policies governing procedures relating to the protection of the clients' confidentiality and governing alcohol and drug use services Document results of assessment, treatment planning, therapy sessions, counseling approach, client education, progress and benefits gained, and ongoing issues/concerns in a timely manner Respond to clients' needs during acute emotional or physical distress by providing crisis intervention Identify client problems requiring resolution Provide individualized counseling, help clients mobilize resources to resolve problems, modify attitudes and values Review treatment plans and modify weekly Obtain authorization for length of stay from the appropriate funding source Facilitate group therapy in the residential program to help clients explore problems and ramifications Facilitate specialized groups utilizing evidence-based curriculums including Complete discharge planning, arrange for continuing care and seek appropriate authorization Agree to and comply with all company policies and procedures, including how to handle patient grievances, proper intake procedures, staff meeting requirements and general code of conduct Assist with general monitoring of residential and detoxification clients Administer Preliminary Breath Test or drug tests as warranted Participate in seminars, training and education for the ongoing acquisition of clinical interventions appropriate to residential treatment, and mandatory in-service trainings Perform any other tasks assigned by the Clinical Supervisor Demonstrate competence in the cultural issues impacting the lives of residents and clients, including consideration of age, gender, sexual orientation, spiritual beliefs, and socioeconomic status Provide an integrated approach to treatment planning based on the clients' strengths, weaknesses, and identified problems and needs for inpatient and outpatient clients
Special Skills/Requirements:
Familiarity with ASAM Patient Placement Criteria Proficient with Microsoft Office software, internet, computer, scanner, printer, fax, copier, & phone Perceptive emotional judgement and decision-making Strong social awareness and compassion for challenging, sensitive interactions
Physical Demands:
Accept some risk to communicable diseases Able to lift up to 25 lbs. occasionally Able to perform repetitive motions using a keyboard and sit for long periods of time with some walking, standing, stair climbing and stooping Able to handle stressful situations which involves crisis intervention for emotional trauma Accept responsibility for own actions, follows policies, procedures, meets commitments & deadlines Able to work cooperatively with all staff and persons served; treat others with respect and promote a positive work environment Able to communicate effectively and select appropriate methods for communication Able to remain alcohol and drug free in the workplace and pass a drug or alcohol screen and background check at any time before and during employment
