Position Summary:
The Community Support Team Qualified Professional/Direct Care Worker is responsible for assisting the CST Supervisor and Team Lead with the coordination of services across all levels of care. The QP/DCW participates in treatment team processes including clinical documentation, education regarding community services, resources and programs, and skill development.
About The Program:
The CST Program provides integrated and community-based support services, offering tailored interventions and resources to individuals coping with mental health challenges and substance use disorders. About You The ideal candidate must: Be currently licensed by the appropriate North Carolina licensure board as a Licensed Clinical Addiction Specialist, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Counselor, Psychiatrist, Licensed Psychologist or a Licensed Psychological Associate, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in mh/dd/sa with the population served OR Have a master's degree in a human service field plus one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a master's degree in a human service field) who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR Have a bachelor's degree in a human service field plus two years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a bachelor's degree in a human service field) who has two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR Have a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a bachelor's degree in a non-human services field) who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling
Key Responsibilities:
Provide direct care to clients within the treatment setting. Participate in the initial development, implementation, and ongoing revision of the PCP. Provide and reinforces skill-building development and strategies to promote independent living as indicated in the PCP. Assist the Team Lead with behavioral and substance use disorder treatment interventions. Assist with the development of relapse prevention and disease management strategies. Consult with identified medical (i.e., primary care and psychiatric) and non-medical providers, engage community and natural supports, and includes their input in the person-centered planning process. Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations. Monitor and document the status of the individual's progress and the effectiveness of the strategies and interventions outlined in the PCP. Assist with the individual's housing search including engaging landlords to rent to individuals and writing reasonable accommodation letters. Assist with rehousing individuals if they are no longer able to stay in their unit due to eviction or risk of eviction. Assist in developing daily living skills to stabilize and maintain housing. Complete functional needs assessments to determine the scope and anticipated outcomes of the service. Equal Opportunity We are an equal opportunity employer.
