Responsibilities:

Responsible for providing direct care services including individual, family, and group counseling, support, and other activities; completes comprehensive assessment, development of treatment and discharge plan of care; maintains all clinical documentation; participation in and facilitation of all family and/or collateral meetings, interdisciplinary treatment team meetings, case conferences or other meetings as required to ensure continuity of care. Completes all required utilization review tasks as assigned. For Outpatient and Residential services/programs transportation to client residence and/or transportation of clients may be required.

Qualifications:

EDUCATION, EXPERIENCE, TRAINING LCSW

preferred;

LPC, LCADC

license for independent practice level required. (If the employee holds one or more of these licenses, primary source verification must be tracked annually). Master's in Counseling, Psychology or Social Work. #LI-ED2 #LI-MA2

The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
