Responsibilities:
Provide comprehensive assessments, evaluations and diagnostic summaries for chemically dependent and dually-diagnosed patients. Develop patient-centered, individualized and comprehensive treatment plans. Document patient progress towards goals in treatment within OASAS and other regulatory standards. Facilitate and provide group therapy. Provide case management services and coordination of care with key stakeholders. Complete discharge summaries and appropriate aftercare planning.
Qualifications:
Excellent written, computer and oral communication skills are necessary. Potential candidate will be able to work well within the framework of a high functioning team. Position requires
Qualified Health Professional:
CASAC, LMHC, LCSW, LMSW, MHC-LP.
