Psychosocial Rehabilitation Counselor

Job Description:

Psychosocial rehabilitation counselors must possess at least a bachelor's degree, offer psychosocial and services that help patients adjust to their social environment. Counselors may utilize individual or group counseling, and they may provide assistance with job training, housing needs, and family support issues. Psychosocial rehabilitation Counselors assesses a patient's current condition and level of social functionality. Then, they develop a treatment plan that helps a patient learn skill sets needed for independent living. Psychosocial rehabilitation counselor monitor patients' progress, help them achieve their social integration goals. Must have some experience working with clients who have ha a mental health diagnosis.

RESPONSIBILITIES

• Maintain confidentiality of records relating to clients' treatment.
• Assess patients for suicide/homicidal risks.
• Fill out and maintain client-related paperwork, including federal- and state-mandated forms.
• Complete documentation required by programs or regulations.
• Teach life skills or strategies to clients and their families.
• Perform crisis interventions with clients as needed.
• Intervene in crisis situations to assist clients.
• Develop and implement treatment plans based on client's needs.
• Evaluate characteristics of individuals to determine needs or eligibility.
• Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.
• Modify treatment activities or approaches as needed to comply with changes in clients' status.
• Modify treatment plans to accommodate client needs.
• Discuss with individual client his/her discharge plans post services.
• Collaborate with other professionals to assess client needs or treatment goals.
• Monitor clients' use of medications.
• Monitors and assesses program members functioning progress in meeting service plan objectives,and advises agency staff and other service providers, both verbally and/or in writing, of member needs to ensure adequate service provision.
• Maintains client case records in compliance with applicable agency and licensure documentation requirements to ensure agency and licensure documentation requirements to ensure effective service delivery, continuity, and quality assurance.
• Participates in program and interagency staff meetings, sharing information with consulting psychiatrist,mental health nurse, case managers, and therapists, as well as other service providers, to enhance effective service coordination and planning.
• Provides and/or facilitates education and support groups for program members in such areas as psychotropic medications, prevocational and life skills, health, and other relevant issues, to promote client independence, effective decision making, and empowerment.
• Performs other duties as assigned by Supervisor, which are consistent with the position and in compliance with agency policies and procedures.
• Plan programs to address community health issues.

Physical Requirements:

• Must be able to life and carry up to 50 lbs.
• Must be able to talk, listen and speak clearly on telephone.

$19.00 - $20.00 per hour
