Rehabilitation Coach Community Connections For Independent Living Clinton Township, MI Job Details 23 hours ago Qualifications Driving Class D Associate's degree Full Job Description Community Connections for Independent Living, LLC provides semi-independent residential programming for individuals who have sustained a Traumatic Brain Injury (TBI). Services are guided by the Occupational Therapy Practice Framework, Positive Behavior Supports, the Interdependence Model, and evidence-based rehabilitation principles aimed at maximizing meaningful outcomes and quality of life. The Rehabilitation Coach is an essential member of Community Connections' interdisciplinary team and plays a key role in supporting residents as they work toward their individualized rehabilitation goals. Rehabilitation Coaches implement and reinforce each resident's Individualized Rehabilitation Plan (IRP) while fostering a therapeutic environment that is safe, respectful, dignified, and supportive of personal choice, autonomy, and self-determination. Their work promotes skill development, community integration, and increased independence through consistent, person-centered support.
Duties and Responsibilities:
1. Promotes residents'/clients' independence within the defined 5 Life Domains of Instrumental Activities of Daily Living, Cognitive Abilities, Social Participation, Education and Work 2. Provides assistance and training to residents/clients in achieving the goals of their Master Treatment Plan in a private home and/or community-based setting 3. Completes all required documentation in accordance to Community Connections' protocols for each resident/client 4. Communicates any changes in a residents'/clients' condition to their lead Occupational Therapist and/or Director of Clinical Services 5. Assists in planning and implementing recreational outings with assigned residents/clients related to their interests and avocations 6. Transports residents/clients to physician appointments and to community and recreational outings 7. Participates in the assessment of a residents'/clients' progress toward identified goals within their Individualized Rehabilitation Plan (IRP) 8. Completes daily physical checks and phone checks for each resident/client as assigned 9. Participates in staff meetings and assigned team conferences 10. Performs duties and responsibilities in a professional, ethical, and timely manner 11. Assumes personal responsibility in providing a safe and therapeutic environment for residents/clients 12. Adheres to Community Connections' policies as outlined in the
Employee Manual Required Qualifications and Education:
1. Must possess an Associate Degree from an accredited institution 2. Must possess a valid Michigan Operators License and meet the driving criteria as outlined in Community Connections' Employee Manual 3. BLS/First Aid Certified through the American Heart Association 4. Complete criminal background check 5. Willingness to complete all required formal training after employed, including but not limited to Certified Brain Injury Specialist (CBIS) training
Supervision:
The Rehabilitation Coach functions under the direct leadership, delegation, and supervision of the Director of Clinical Services
