Community Skills Program, a division of Success Rehabilitation, Inc., is seeking a Cognitive Rehabilitation Therapist to provide vital, one-on-one support in Trenton, NJ, and throughout Mercer County. About the Role This role is designed for an effective communicator and problem-solver who enjoys working independently. You will work directly in the client's home and community, facilitating functional changes to maximize independence.

Clinical Focus Cognitive Foundations:

Attention, concentration, and memory.

Executive Functioning:

Planning, organizing, reasoning, and judgment.

Information Processing:

Speed and accuracy of understanding.

Self-Regulation:

Impulse control and behavioral patterns. Your Impact Reinforce or establish previously learned patterns of behavior. Develop new cognitive mechanisms to compensate for impairments. Implement interventions to ensure client safety and community interaction.

Responsibilities Assessment:

Participate in standardized evaluations to determine individual strengths.

Treatment Planning:

Implement individualized programs based on assessment data.

Direct Service:

Conduct sessions in home/community settings per the IPD.

Collaboration:

Coordinate with the treatment team and complete documentation by deadlines.

Qualifications Education:

Bachelor's degree in Psychology, Social Work, Education, or related human services field.

Certification:

CBIS (Certified Brain Injury Specialist) preferred or CBIS eligible.

Experience:

Minimum 1 year working with adults with TBI or intellectual disabilities.

Requirements:

Valid Driver's License and a reliable vehicle (50% travel required). Benefits 401(k) Flexible schedule Paid time off (PTO) Willingness to travel: 50% (Required)
