Transition Facilitator #26-47

Full Time, Oahu The Transition Facilitator is responsible for providing therapeutic transition services to young people with serious emotional and behavioral problems and their family with in the NOP program. These services may include, but are not limited to, teaching concepts from the Teaching-Family Association (TFA) model, Common Sense Parenting (CSP), Personal Life Planning (PLP), and the Transition to Independence Planning (TIP) model. The Transition Facilitator works with young people by using skill building, relationship development, and problem solving techniques; conducting informal individual and group counseling; overseeing the supervised visits with natural family/ caregivers; and teaching independent living skills. Independent living skills focus in the transition domains of employment and career, educational opportunities, and community life-functioning. The Transition Facilitator must demonstrate an active commitment to Catholic Charities Mission and strive to incorporate the four Core Values in all aspects of daily work.

EDUCATIONAL & WORK REQUIREMENTS

Must have a Bachelor's Degree in Social Work or related human services field and at least three (3) years experience working with troubled youth or young adults.

EMPLOYMENT CLEARANCE REQUIREMENTS

TB Clearance, CAN Clearance, FBI Clearance, Hawaii/National Sex Offender Clearance, Traffic Abstract, Hawaii State Criminal Check, Driver's License, Access to Vehicle with Current Valid Registration, Safety Check, and No-Fault Insurance, 3-Year Employment History, Education Verification.
