Position Responsibilities The Emergency Shelter Case Manager/Youth Speciliast has the responsibility to provide intensive, coordinated case management services to all runaway, homeless, and at-risk youth and their families who enter and exit The Connection Youth Services' Emergency Shelter. The Emergency Shelter Case Manager facilitates a wraparound approach to case planning with youth, their natural support system, and community-based services such as community mental health, school administration, and other human service agencies. Additionally, this position provides support and team leadership to the shelter's Youth Specialists who provide essential direct care to shelter residents. General Case Manager Responsibilities Commitment to Diversity, Equity, and Inclusion, promotion of an environment that is welcoming to all, and positive team contribution Knowledge and compliance with LFC agency policies, procedures, and privacy practices Knowledge and compliance with all TCYS program policies and procedures Participate in the agency's 24-hour on-call line that provides crisis intervention, referrals, and initial screening of potential TCYS clients Engage with youth and families utilizing evidence-based frameworks including, but not limited to, positive youth development, harm reduction, trauma-informed care, and strengths-based perspective Participate in staff meetings, supervision, and training opportunities Participation in community collaboration and workgroups as appropriate Participation in agency fundraiser activities Maintain general cleanliness, organization, and safety of facilities Provide intensive, coordinated case management and support services to youth and families Facilitate and document plan and goals meetings with youth and families Provide aftercare and home-based case management services to youth and families Assist youth and families to develop and implement treatment and discharge plans Assist with monitoring medication administration of youth; coordinate with parents and health care providers as needed Complete face-to-face intake with each client within the required guidelines Additional responsibilities may be assigned by the LFC Executive Director, RHY Program Director, and/or LFC Board of Directors Data Responsibilities Maintain

database on all assigned clients Knowledge and compliance with all MDHHS, FYSB, and other funders' reporting requirements Maintain a chart for each client as required within time requirement guidelines Develop and maintain community-based resource file of various agencies and services that assist youth and families Assist with completing and compiling 90

- and 365-day follow-up interviews Compile youth pre and post survey responses; share program improvement suggestions based on data compilation Compile any additional data to monitor and assess agency and runaway/homeless youth trends Youth Specialist Responsibilities Compliance with all policies and procedures regarding client care and supervision Youth Specialist shift coverage and compliance with all Youth Specialist responsibilities during assigned shift Supervise youth residents to keep within the guidelines of shelter rules and expectations Plan and supervise appropriate educational and recreational activities with youth Knowledge of all daily log information Completion of all necessary documentation during assigned shift Timely and open communication with supervisor of significant information, unusual occurrences or general concerns regarding residents, staff, or the facility Qualifications The Emergency Shelter Case Manager/Youth Specialist will be committed to promoting an inclusive work environment and positive team contribution.
The preferred candidate will have:
Bachelor degree in the human services field Two years of experience working with youth and families Valid driver's license and successful completion of all required criminal checks and health screenings CPR/First Aid Certification (agency may provide reimbursement for completion) Must be willing to work evenings and weekends as needed Benefits and Compensation The Emergency Shelter Case Manager/Youth Specialist is a full-time, non-exempt position.
