SUMMARY OF JOB RESPONSIBILITIES
The bereavement counselor is responsible to plan and implement interventions to meet the needs of families/caregivers from Harbor Hospice for thirteen months following the death of the individual Hospice patient. The bereavement counselor may also assist in providing grief and loss services provided by hospice to the community-at-large for all age populations, including children. ESSENTIAL FUNCTIONS AND DUTIES
(other duties may be assigned) Always maintains the confidentiality of patient and employee information, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations. Is knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job. Knowledge of Hospice concept and philosophy Assesses the need for grief counseling and supportive services of the bereaved hospice caregivers and community referred bereaved clients Formulates, implements, evaluates and updates as appropriate the bereavement plan of care Provides bereavement counseling, information, and referral services to bereaved Hospice families Provides counseling and bereavement information and referral services to callers from the community Utilizes knowledge of community needs and resources to assist clients and to develop and expand hospice bereavement services Implements specialized bereavement services such as church-based bereavement training, Camp Courage, specialized education/support groups, memorial services, etc Meets productivity standards and targets as set by the agency and the clinical manager Directs and supervises the activities of volunteer staff involved in bereavement services including support groups, mailings, etc Develops and provides educational presentations on bereavement topics to staff, volunteers and the community-at-large
QUALIFICATIONS
Graduate of a Master's program in social work accredited by the Council on Social Work Education. Current State of Michigan Social Work License (LMSW or LLMSW) or Licensed Professional Counselor (LLPC or LPC). Current driver's license. Previous experience with grief and loss counseling preferred
