Job Description

Adoption Connection PA is a dedicated non-profit organization specializing in foster care and adoption services, licensed by the Commonwealth of Pennsylvania. We are committed to providing compassionate support and comprehensive care to children and families in need.
Connecting Home:
a reunification preservation program is designed to provide support and guidance to parents who are receiving services after being reunified with children from the foster care system and to actively engage parents in their own growth and healing while providing a safe, stable, and nurturing environment for their children. Job Expectations Conduct through assessments of the family's strengths, needs, and risks to develop a family preservation plan. Monitor family progress and provide ongoing support. Connect families with community resources and support services. This may include access to housing assistance, mental health services, substance abuse services, parenting classes, childcare, employment support, and other social services. Advocate for the rights and needs of the family within the child welfare system and other service systems. Provide crisis intervention and support during times of heightened stress or instability within the family. Collaborate with other professionals and agencies involved in the family's care, such as mental health providers, educators, healthcare professionals, and legal advocates. Maintain accurate and up-to-date documentation of all interactions, assessments, family preservation plans, and progress made by the family. Prepare and submit reports to supervisors, and other team members to ensure accountability and compliance with their county Family Service Plan. Qualifications Bachelor's Degree preferred in a social services field or 3 years related experience Current driver's license Ability to past all child abuse clearances Proven experience working with children and families in clinical or community settings.
