Job Description
Job Overview Come join our brand-new Behavioral Health facility! The Clinical Social Worker position is a pivotal member of the Inpatient Treatment Team. This position requires a dynamic individual with a strong background in inpatient care and compliance with PA healthcare regulations. This candidate will ensure the delivery of high-quality patient care while adhering to established protocols and standards. This role demands trauma-informed therapeutic skills, knowledge of surrounding resources, and excellent communication. Experience
- Proven experience providing Social Work/Case Management skills in a healthcare setting.
- Strong knowledge of clinical and medical terminology.
- Experience with clinical documentation timelines and compliance with HIPAA regulations.
- Excellent communication skills, both verbal and written, with the ability to lead discussions and present information clearly.
- A commitment to providing compassionate trauma-informed patient care while maintaining operational efficiency.
- Master's degree in social work required. Inpatient Social Worker Duties and Responsibilities
- Conducts patient Biopsychosocial Assessments within 72 hours of admission to the inpatient unit to document patient history and identify preliminary issues for treatment.
- Works closely with the patient, patient's family/significant others, and entire treatment team to develop a Master Treatment Plan with measurable goals within 72 hours of admission to the inpatient unit.
- Updates the patient treatment plan at least every 7 days and/or during Treatment Team.
- Collaborates with the patient and their family/significant others to develop a Resilience Action Plan within 72 hours of admission.
- Completes a Case Management Screening Assessment to identify immediate and long-term needs.
- Starts the Discharge Planning process at admission by working with the patient and their family/significant others to identify the patient's aftercare needs, support preferences, and explore potential referrals.
- Attends twice a week Treatment Team meetings to provide a social work perspective to total case management of the patient by discussing clinical treatment progress, case management needs, and aftercare plans.
- Maintains contact with the patient's family/significant others/outside supports at least 2x/week via telephone or in-person.
- Provides positive customer relations in dealing with patients, families, physicians, peers, outside agencies, and referral sources.
- Communicates with the physician to report patient progress, needs, concerns, and potential barriers to discharge.
- Provides daily Clinical Therapy Group for 1 hour Monday-Friday, including Psychoeducational, Cognitive Behavioral, Trauma-Informed, Person-Centered, Motivational Interviewing, Mindfulness, Grounding, and Somatic Interventions.
- Provides short-term individual and family therapy, resource planning, and crisis prevention and intervention.
- Develops and maintains a thorough knowledge of referral resources throughout the service area.
- Maintains accurate and timely records, including Clinical Progress Notes, Collateral Contact Notes, Social Work Notes, Group Notes, etc.
- Provides back-up services for incoming assessment and referral calls as needed.
- Maintains a professional approach with confidentiality, ensuring the protection and privacy of health information as attained through written, electronic, or verbal disclosures.
- Acts as a trauma-informed leader during crisis situations.
- Provides trauma-informed training for new hire orientation and on-going for employees of Three Rivers Behavioral Health.
- Participates in planning and implementing quality improvement activities and plans.
- Helps other departments/programs and completes other duties as needed.
