Job Description

Job Summary We are seeking a caring and motivated Social Worker to join our skilled nursing team and provide support to residents and their families throughout their healthcare journey. The ideal candidate will assist with care planning, discharge coordination, psychosocial assessments, and connecting residents with appropriate resources and services. This individual should be highly organized, an excellent communicator, and passionate about advocating for the emotional and social well-being of others. Responsibilities Provide emotional support and psychosocial services to residents and families Complete social service assessments, care plans, and required documentation Assist with admissions, transitions of care, and discharge planning Advocate for resident rights, dignity, and quality of life Coordinate with interdisciplinary team members to support resident wellbeing Facilitate family meetings and communicate updates as needed Assist residents and families with community resources and advance care planning Maintain compliance with state, federal, and facility regulations Qualifications Degree in Social Work or related field preferred Long-term care or skilled nursing experience preferred Strong communication and interpersonal skills Ability to handle sensitive situations with professionalism and compassion Knowledge of discharge planning, care coordination, and resident advocacy Organized, detail-oriented, and able to manage multiple priorities We believe our employees are one of our greatest strengths and are proud to offer a supportive environment focused on growth, appreciation, and work-life balance.
