We are also seeking an experience for Per Deim social worker to provide coverage support.

Expectations :

Ability to step in, take direction, and function independently. Strong clinical judgment and efficiency. Dependable, responsive, and flexible Maintains professionalism and teamwork in a fast-paced setting. Qualifications CSW or MSW required. Minimum three years experience in subacute and long-term care. Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities. Responsibilities for a Social Worker CSW/MSW Assists residents with emotional adjustment to facility. As directed, interview new residents and records, social history, and assessments. Completes the social services portion of the MDS, Care planning, baseline, etc. Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc. Writes summaries and progress notes as directed. Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community. Assists with discharge planning. Follows up after discharge. Conducts required Cut Letters Writes discharge summaries as assigned. Develops a professional relationship with each resident and family. Educates residents/ representatives on Advanced Directives and POLST. Insurance updates and appeals Transitions Setting up and maintaining the household model Adheres to all Social Services Regulations. Benefits Referral program 401(k) matching
