Job Description
Posting Details This posting will be used for ongoing recruitment and may close at any time. Applicant lists may be screened more than once. Why does the job exist? This position will provide effective advocacy for long-term care residents and acts as a liaison with internal and external entities, ensuring resident rights are not violated and resident and family concerns are addressed. Will provide comprehensive resident assessments, as well as care and discharge planning. How does it get done? Advocates and assists resident to attain, or maintain his/her highest level practicable well-being Perform medically related social services needs in accordance with the State, Federal and Veteran Regulations, Complete comprehensive resident assessments and care plans Document in computer resident needs, requests, appointments and other necessary information. Reviews grievances and resident/family concerns and takes appropriate steps towards resolution. Assess for Trauma and develop care plans to meet residents' needs and monitor for PTSD symptoms Participate in pre-admission assessment process Facilitate discharge planning process with resident and Interdisciplinary team members. Participate in Psychopharmacology Review team to assess resident emotional, mental and cognitive impairments and related medication. Who are the customers? Residents of a Long-Term Care Facility. Resident who are admitted for short term skilled care with anticipation to discharge back to community. Work with families and/or legal representatives, insurance care coordinators, community medical providers, community therapy providers, medical equipment providers, and veteran's services. Minimum Qualification Associate's Degree in Human Services or Social Sciences and two (2) years experience in mental health and/or substance abuse counseling. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience. Employment Requirements This position is designated as safety sensitive. Employment is subject to pre-employment and random testing in accordance with all terms and conditions of federal and state law, rules and regulations in relation to alcohol and/or drug testing. Will be screened for tuberculosis (TB) upon hire. Employment is subject to criminal background check pursuant to New Mexico Caregivers Criminal Screening Act (Sections 29-17-2 through 29-17-5 NMSA 1978 and 7.1.9.1 through 7.1.9.11 NMAC) and is conditional pending results. Working Conditions Works in office area(s) as well as throughout the nursing service area (i.e., nurses¿ stations, resident rooms, etc.). Moves intermittently during working hours. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government agencies/personnel, etc., Is subject to hostile and emotionally upset residents, family members, personnel, and visitors. Communicates with the medical staff, nursing personnel, and other department directors. Works beyond normal working hours and on weekends and holidays when necessary. Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). Is subject to exposure to infectious waste, diseases, conditions, etc., State of New Mexico employees may be impacted by the Public Health Order dated December 19, 2022, which requires facilities licensed or certified by the Centers for Medicare and Medicaid Services (¿CMS¿), including all hospital types, long-term care facilities, nursing homes, hospice facilities and rehabilitation facilities, as well as assisted living facilities and adult day care settings, to adhere to all COVID-related requirements prescribed by CMS, including, but not limited to masking and staff vaccination. Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
