Job Description
Overview Social Worker Samaritan Daytop Village is a leading human services organization in New York State serving over 38,000 New Yorkers annually across more than 60 locations through Transitional Housing, Community Based Services, and Residential and Outpatient programs. First Steps Intensive Crisis Stabilization Center (ICSC), operated by Samaritan Daytop Village in Rockland County, is a 24/7 program dually licensed by OMH and OASAS to provide rapid, short-term support for children ages 5+ and adults in mental health or substance use crisis. Our multidisciplinary team utilizes a trauma-informed, harm-reduction framework to stabilize individuals and connect them with long-term care while avoiding unnecessary emergency room visits. We are seeking a licensed Social Worker (LMSW or LCSW) to join a multidisciplinary team as a clinical advocate for individuals navigating behavioral health and substance use challenges. This role focuses on providing trauma-informed crisis intervention, comprehensive biopsychosocial assessments, and life-changing safety planning. This position offers a vital opportunity for a dedicated clinician to coordinate complex discharge plans and empower individuals on their unique path to recovery within a fast-paced, integrated care environment. Responsibilities Conducts comprehensive clinical assessments to understand each individual's mental health, substance use history, and personal strengths Screens for safety risks and works closely with clients to create practical, personalized safety plans using evidence-based tools Uses clinical frameworks to determine the best next steps for a client's continued treatment, recovery, and level of care Offers immediate support and crisis counseling using evidence-based approaches like Motivational Interviewing and trauma-informed care Leads the discharge process by connecting clients to essential community resources, such as housing, outpatient clinics, and primary care Works as a key member of an integrated team that includes doctors, nurses, and peer specialists to provide well-rounded care Keeps accurate and timely records of patient progress within the electronic health record system in accordance with regulatory standards Joins regular team meetings and case conferences to ensure every client is receiving the best possible support and advocacy Follows all privacy and healthcare laws, including HIPAA and 42 CFR Part 2, to protect patient rights and maintain professional standards Welcomes and supports individuals who may be experiencing acute distress or active substance use symptoms with empathy and respect Coordinates discharge planning by providing vital linkages to outpatient mental health, SUD treatment, MOUD, benefits, health homes, and community supports Performs clinical duties within the scope of licensure to ensure a safe, organized, and person-centered environment Qualifications Master of Social Work (MSW) from a CSWE-accredited program Active New York State LMSW license required; LCSW strongly preferred Preferred minimum of one (1) year of post-MSW experience, ideally in crisis services, behavioral health, or co-occurring treatment Knowledge of clinical frameworks includingDSM-5-TR, ASAM
Criteria, and the Stages of Change model Clients in the center are experiencing significant crises; the ability to effectively engage clients in crisis is required. Proficiency in Electronic Health Records (Avatar preferred) and Microsoft Office Suite Familiarity with the New York State behavioral health system, including Medicaid Managed Care andOMH/OASAS
regulations Proven ability to engage with individuals from diverse racial, ethnic, and socioeconomic backgrounds with empathy and clinical skill This is a 24/7 program, so schedules may include non-traditional hours. Some flexibility to work outside of regularly scheduled hours may be needed to cover unexpected circumstances. Strong organizational and documentation skills with the ability to manage a caseload in a fast-paced environment Additional Information This is a full-time exempt role with an annual salary of $70,000-$79,568 with a full benefits package. Benefits include medical, dental, and vision insurance, 25 days of paid time off, access to a retirement account with an employer to match, and tuition reimbursement.
