Job Description

The heart transplant social worker delivers specialized psychosocial care to patients across the continuum of advanced heart failure, mechanical circulatory support, and heart transplantation. This role requires strong clinical judgment, efficiency, and the ability to perform effectively under pressure while maintaining professionalism, sound decision-making, and reliable follow-through. As a core member of the multidisciplinary team, this position supports patients and caregivers from initial evaluation through candidacy review, waitlist management, transplant hospitalization, and lifelong post-transplant follow-up. The social worker provides psychosocial support in both inpatient and outpatient settings, addressing clinical, emotional, logistical, and caregiver-related needs while helping patients navigate insurance, disability, and financial assistance resources with clarity and empathy. This role calls for advanced clinical judgment, strong psychosocial assessment skills, and the ability to complete urgent, high-stakes evaluations in a fast-paced environment. It also requires knowledge of psychosocial complexities associated with long-term device management, transplant readiness, and patient stability. At Mayo Clinic Arizona, transplant social workers are valued as essential clinical partners within a highly collaborative, nationally recognized transplant program. extension program. A MSW degree from an accredited Master of Social Work Program and 2 years of post-MSW social worker experience in a hospital setting are required. License/Certification Active Licensed Master Social Worker (LMSW) license through the Arizona State Board of Behavioral Health Examiners is required Additional Qualifications Transplant social work experience preferred Experience working with insurance-driven care pathways or specialty medical programs preferred Certification in Transplant Social Work (CCTSW) preferred, or willingness to pursue. Strong work ethic and ability to manage a fast-paced transplant caseload. Advanced clinical skills with the ability to work under pressure without compromising integrity or judgment. Strong understanding of psychosocial patterns in chronic illness and transplant populations. Excellent organizational, communication, and problem-solving skills. Ability to navigate ambiguity, prioritize effectively, and collaborate across disciplines. International Degrees A detailed equivalency evaluation is required, demonstrating a U.S. equivalent degree. Evaluation must be completed by an organization listed as a member of the National Association of Credential Evaluation Services (NACES) (www.naces.org) or the Association of International Credential Evaluators, Inc. (AICE) (www.aice-eval.org).
