• Provides outstanding customer service
• Maintains HIPAA standards related to Privacy and Security
• Provides a broad range of social services to individuals with HIV/AIDS and their families.
• Assesses to determine eligibility for the Ryan White Program and enrolls and recertifies as appropriate. Develops plans and assesses effectiveness.
• Links to community resources for assistance. Participates in community consortium meetings.
• Collects and maintains data and completes and submits required reporting
• May work nights and weekends, participate in health fairs, serve on committees, make presentations, or respond to disasters.
• Responds to Public Health Emergencies Minimum Qualifications Bachelor's degree in a related field from an accredited college or university, and Two years of experience in social service delivery.

• Public Health experience
• Bachelor's or Master's degree in Social Work
• Experience working with individuals with

HIV/AIDS

• ADAP experience
• Case management experience
• More than two years of experience in social service delivery

Highlight any experience related to the Preferred Qualifications . This position may require driving company vehicles (Valid driver's license required) and working some evenings and weekends during special events such as health fairs and responding to disasters.
