Staff Nurse (RN), Home Health - Casual/PRN

We're seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our team! As a Home Health RN, you will provide high-quality, patient-centered care in the comfort of patients' homes. You will work closely with patients and their families to develop and implement individualized care plans, ensuring optimal health outcomes and quality of life. Casual/As-Needed -- flexible schedule!

Responsibilities:

Conduct initial patient assessments and develop comprehensive care plans. Administer medications and treatments as prescribed by physicians. Monitor patient progress and adjust care plans as necessary. Educate patients and their families about their health conditions, treatment plans, and self-care techniques. Collaborate with other healthcare professionals to coordinate care. Document patient care accurately and in a timely manner. Provide emotional support and guidance to patients and families. Adhere to all regulatory and safety guidelines.

Qualifications:

Current RN license in Pennsylvania, or eligible. 2 years of experience in home health, geriatrics, or a related field preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Valid driver's license and reliable transportation to commute throughout Warren County
