Duties Observe and monitor patients' conditions regularly, documenting vital signs and changes Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support with tools like Hoyer lifts. Support patients with dementia or Alzheimer's by implementing specialized memory care strategies and providing compassionate engagement. Help with meal preparation, feeding, light cleaning, laundry, and maintaining a safe, hygienic environment in accordance with HIPAA regulations. Assist with medical documentation and ensure compliance with healthcare standards while respecting patient confidentiality. Work closely with nursing staff to observe patient responses to treatments and report any concerns promptly. Experience Prior experience in long-term care facilities such as nursing homes or assisted living communities is highly preferred. Experience working with individuals with developmental disabilities or those requiring hospice or dementia care is a strong plus. Familiarity with medical terminology, vital signs measurement, medication administration, and use of EMR/EHR systems is essential. Demonstrated ability to provide patient care in home health or community settings is desirable. Certification as an STNA, CNA, or HHA is required; CPR certification is strongly preferred. Knowledge of hospice care protocols, dementia-specific techniques, and working with individuals with disabilities will enhance your effectiveness in this role.
