SCHEDULE TYPE

Every Other Weekend Shifts (Saturdays + Sundays) 4-8 hour shifts per day [8-16 hrs/wk] 2-day weekend availability is required If additional hours are desired, employee will be given first priority of weekday (or add'l weekend) cases to quickly secure total desired hours per week.

LOCATION

Magnolia, Ohio and surrounding areas in Stark County, Ohio Willingness to travel to / from Magnolia for this initial client is required. If additional cases/hours are desired, typical travel to clients throughout the county and/or up to at least a 30-minute radius (from your home) is often required. Up to 45- or 60-minute client travel radius (from your home) is highly desired but not required.

COMPENSATION :

$15 - $19 per hour Pay Rate based on certifications, experience, schedule and travel availability, work history, references, and overall fit.

POSITION SUMMARY

We are seeking a highly-skilled, compassionate, and professional Home Health Aide (STNA or CNA highly desired | experienced HHA, CHHA, DSP, or PCA will also be considered) to join our home health agency and provide hands-on personal care and support to clients in Magnolia, OH and surrounding areas of Stark County. Responsibilities Personal Care & Activities of Daily Living (ADLs) including safe transfers including Hoyer Lift (minimum of 1 year experience; 2+ desired) as well as mobility and positioning support Support safe use of assistive devices (walkers, wheelchairs, lifts) Nutrition & Meal Support Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation Basic Health and Safety Monitoring Care Plan Implementation Dementia & Behavioral Support (if applicable) Pro-active observation and excellent electronic documentation

Position Reports to:

RN Case Manager / Program or Regional Administrator A successful DSP provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.
