Targeted Case Manager

Children's Center provides many early intervention and family support services including case management for children, birth through age 21, and their families. The Children's Targeted Case Manager role is to be the life-line and link between families and the services that they need. Targeted Case Managers help families navigate the world of raising children with special needs by assessing their needs, creating plans, connecting families to resources, and collaborating with other providers. The preferred candidate must be able to work independently and as part of a multi-disciplinary team. A Bachelor's degree in Social Work or related field is required; must have LSW or be eligible to apply for conditional social work license through the State of Maine. 1 to 2 years experience providing case management or in-home services to families preferred. Required skills include excellent communication skills, ability to establish rapport, complete assessments, develop and evaluate plans, and accurately document interactions using a family strength-based approach. Travel required; a reliable vehicle, valid insurance and valid driver's license are a must. Knowledge of Early Intervention and experience with children with special needs preferred.
