Job Description
Provide information regarding resources and programs for older people and/or their caregivers, and link them to appropriate services, if needed. Complete intake assessments for those services that the CSI Office on Aging provides funding and services for through contracted providers such as Respite and Home delivered meals. Maintain and develop client, resource information, community partners referral information.
Duties & Responsibilities
Information & Assistance
Secure resource information, record information, disperse information and assist older people and their caregivers.
Screen all clients and their caregivers, assessing their needs and helping identify their problems and capacities.
Provide information regarding service options and choices available; and link individuals to such options and choices.
Maintain a current and accurate database, containing required information on all consumer contacts.
Maintain a current and accurate database of available resources.
Develop and maintain good working relationships with service organizations, senior centers, churches, civic groups, and other provider groups in service areas.
Follow-up at least 10% of inquiries to determine the outcome of referrals and if referral resources were appropriate to meet identified needs.
Other Responsibilities
Attend required Meetings.
Participate in training to enhance skills.
Perform such other duties as assigned by the Director.
Qualifications/Skills
Applicant must possess:
Demonstrated listening and other people skills, including an interest in and ability to work and communicate with older people, by telephone and in person.
Ability to maintain a courteous, professional, and effective working relationship with all employees, vendors, clients, and other representatives of external organizations.
Demonstrated skills in computer database management, and other software programs such as Word and spreadsheets.
Ability to work independently, organize and prioritize tasks, and assure all work is completed accurately, within time allotted.
Must have or develop a good understanding of the human services delivery system in south central Idaho.
Bilingual English/Spanish is a plus.
General Requirements
This position requires a minimum of a high school diploma or equivalent. An associate degree in a human resources or business administration is preferred.
Working Conditions and Physical Requirements
Information & Assistance Part Time Specialist is required to work 19.5 hours per week. The work is predominately telephone and computer related. There is contact with the walk-in public. Occasional travel out of the area may be expected to attend conferences and training courses, as approved by the Director of the Office on Aging.
