reviews $43.36•$54.19 an hour Completion of all resident Minimum Data Set and Care Plans and Interdisciplinary Summaries, both hard copy and computer data entry of same. Initial, Quarterly, Annual and special Minimum Data Set and Care Plans in accordance with State and Federal guidelines. Set up and conduct care plans.
ESSENTIAL JOB FUNCTIONS
Complete initial Minimum Data Set/Care Plans and physical assessment on all new admissions. Conduct all Initial, Quarterly, Annual and special Minimum Data Set/Care Plan reviews and provide Interdisciplinary Summary for same. Provide Director of Nursing and Department Heads with schedule for all Care Plan reviews. Primary contact to residents and families to notify of upcoming Minimum Data Set review. Sign off on hard copy and assume responsibility for computer data entry of same in timely manner as authorized by State and Federal guidelines. Complete HCFA sheets monthly and provide copies to Director of Nursing and Human Resources. Follow through with Clinical Coordinator on all Care Plan recommendations. Keep current on all changes related to Medicaid/Medicare and PPS. Conduct chart audits to ascertain that documentation is present for billing. Will work "the floor" for med pass or treatments one weekend per month and one holiday per year; and continue to take call (phone). Able to perform Essential Functions with or without reasonable accommodations.
NON-ESSENTIAL FUNCTIONS
Work "the floor" during times of staffing emergency when requested by Clinical Coordinator or Director of Nursing. Attend seminars related to MOS, PPS and HCFA changes to maintain proficiency. Be primary contact between Nursing Department and Accounting Department. Assist with all new admissions and discharges to see that medical record is accurate and complete. Assist medical records with "closing out" chart of discharged residents. Assist Director of Nursing with Quality Assurance when requested.
