$50/hour About the Role Horizon Healthcare Staffing is seeking an experienced MDS / Care Plan Coordinator (RN) for a temp-to-perm opportunity with one of our long-term care partners. This role is essential in ensuring accurate resident assessments, care planning, and regulatory compliance. Opportunity to transition into a permanent role Competitive pay at $50/hour Dedicated recruiter support Access to top healthcare facilities Key Responsibilities Complete Minimum Data Set (MDS) assessments and comprehensive care plans for all residents Perform initial, quarterly, annual, and significant change assessments Develop and update interdisciplinary care plans and summaries Coordinate and attend IDT (Interdisciplinary Team) meetings Ensure compliance with state and federal regulations Maintain accurate documentation for Medicare/Medicaid and billing purposes Collaborate with nursing staff and department heads on care plan implementation Conduct chart audits to ensure documentation accuracy and completeness Communicate with residents and families regarding care planning Additional Responsibilities Assist with admissions and discharges Support quality assurance initiatives Stay current on PDPM, CMS, and regulatory changes Participate in facility trainings and meetings as needed Qualifications Active RN license (state-specific) 2+ years of clinical experience in long-term care or skilled nursing Prior MDS experience required Familiarity with PointClickCare or similar EMR systems Strong organizational and communication skills AANAC certification preferred Apply Today!
