Job Overview:
Coordinates the delivery of high quality health services to children and families by completing accurate and comprehensive health screenings, tracking the health needs of children, ensuring that all health requirements are met, assisting families with the referral process, and obtaining follow up care services. Completes timely and accurate data entry to maintain training, inkind, and health requirements.
Essential Job Responsibilities and Duties:
Coordinates the delivery of high quality health services by obtaining and/or conducting age appropriate health and sensory screenings according to Head Start Performance Standards and the Early Periodic Screening and Diagnostic Tool (EPSDT). Assures the implementation of health services by verifying that each child's records are current for immunizations, screenings, medical treatments, follow up services, and general health status by entering health data, tracking program activities, developing Individual Health Care Plans and 6029's by communicating with staff, families, and health care providers. Coordinates health services by providing education, advocacy and referrals to and communicating with staff, health care providers, and parents to ensure the overall health and safety of enrolled children, including follow up services. Assists families in eliminating barriers to high quality health services. Completes essential data entry functions in the areas of application, enrollment, health requirements, inkind, staff training, and other areas as assigned, including inputting and retrieving data, in a timely and efficient manner. Promotes the healthy growth and development of children on an as needed basis by supporting classroom staff in implementing developmentally appropriate activity plans in the center setting, by using expected behavior management practices, and maintaining a clean, safe, and well-organized learning environment. Maintains and implements professional and technical knowledge by attending training, establishing professional networks, and documenting training attendance. Promotes a positive agency image and credibility by adhering to our code of conduct. Assures effective risk management practices by identifying and reporting potential safety, ethical, and legal concerns. Contributes to team effort by accomplishing program goals and completing other duties as assigned.

Will work all assigned hours and report to work on time. Will meet all time frames and due dates without reminders. Will make appropriate use of leave time to balance personal, agency and workload needs.
Qualifications:
EDUCATION
: High school diploma or equivalent
License Requirements:
Current, valid driver's license and clean driving record.
EXPERIENCE
One year experience in a health care or early education setting with responsibilities related to health screening, referral and follow up care. Candidate must possess strong communication, organization, technology, and data entry skills.

Computer work is an essential function of this position and vision strain is a potential exposure. This position requires the ability to drive a personal or agency owned vehicle.
