Minimum Data Set Coordinator Toledo, OH Job Summary:
A highly recommended skilled nursing facility is seeking a skilled and detail-oriented Minimum Data Set Coordinator to join our team in Toledo, OH. As a critical member of our healthcare organization, you will play a vital role in ensuring the accuracy and completeness of patient data, utilizing the Minimum Data Set (MDS) to capture and report patient information. Responsibilities:
Collect and analyze patient data, including demographics, medical history, and treatment plans Utilize the MDS to ensure accurate and complete data capture and reporting Develop and maintain complex spreadsheets, tables, and reports to track patient data Collaborate with healthcare teams to gather and verify patient information Identify and resolve data discrepancies and inconsistencies Maintain confidentiality and adhere to HIPAA regulations Participate in quality improvement initiatives to improve data accuracy and patient outcomes
Requirements:
2 years of experience in a healthcare setting, with a strong understanding of patient data collection and reporting High school diploma or equivalent required; associate or bachelor's degree in a related field preferred Strong analytical, organizational, and communication skills Proficiency in Microsoft Office, particularly Excel Ability to work independently and as part of a team in a fast-paced environment Strong attention to detail and ability to maintain confidentiality
