Through housing, employment support, and essential wraparound services, we equip at-risk veterans and their resilient families to rebuild stable, independent lives. THE OPPORTUNITY
We are looking to fill a critical role as a Supportive Services for Veteran Families (SSVF) Intake & Exit Coordinator for our Wheaton, IL location. This program position, reporting to the SSVF Program Manager, is responsible for the intake and exit of all clients referred to and in the SSVF Program per SSVF Grant standards and requirements. The SSVF Intake and Exit Coordinator should represent MSHV's core values - emphasizing compassion and individualized care. The position will be responsible to lead and develop support for the following areas: client intake, client exit, client outreach, and diligence to adhering to the SSVF Program Guide. WHAT YOU'LL BE DOING
Compose and disseminate information about the program, including but not limited to the success stories of clients, drafting final proofs of success stories, and program description for funders and social media. Creating and tracking best practice measurements in the SSVF and Housing First space. Drafting and submitting annual budget documents and analysis for the program on a quarterly/annual basis. Stay abreast of leading topics in the field of veteran housing services to provide trainings to staff and generate new content in community forums. Seek ways to represent MSHV at community organizations to raise awareness regarding the program. Assist MSHV in five-year strategic goals to secure more affordable housing with supportive services (Hero Homes). SSVF Client Services. Conduct screening and assessment for literally homeless veterans and their families. Conduct regular outreach to emergency shelters, Veterans Assistance Commissions, VSOs, VA Medical Centers, etc. to identify and engage literally homeless veterans. Provide assessment, outreach, housing advocacy, eligibility-determination, tenant education, and housing stabilization services to families accepted into the SSVF Program. Assists clients with obtaining mainstream and veteran specific benefits. Must be able to accurately enter client data and information into an established HMIS system. Works with MSHV SSVF
team as needed to cultivate relationships with current and potential landlords as required to maximize units available to families. Provide aftercare services as needed to families who have become stabilized in housing and exited the program. Represent MSHV to local, state, and national organizations as assigned. Work towards empowering clients to be productive members of the community; focusing on their unique skills. Collaborate with MSHV SSVF
Team as needed to quickly coordinate access to services. Coordinate with the MSHV SSVF
Team to exit clients including the completion of MSHV exit surveys and SSVF exit survey registration and completion. Understand the full range of MSHV's services and demonstrate the ability to assess client(s) needs to access services as appropriate. Demonstrate ability to effectively manage multiple tasks at the same time due to the often-urgent needs of clients served and the multi-tasking nature of the job. Respond to emergencies with immediacy using appropriate crisis intervention techniques. Prepare and maintain accurate confidential case records, paperwork, and fiscal recording as required. Network, when appropriate, with the community at large, depending on client needs. Advocate for clients and make appropriate professional contacts. Participate in Continuums of Care and By-Name List meetings as assigned. Planning & Organizing are key. We need someone who diligently attends to details and pursues quality in accomplishing tasks. This means someone who can initiate adherence to the SSVF Program Guide and seek cohesion among the SSVF team supports. This person needs to be action oriented. That means actively seeking early completion of deadlines, creatively developing solutions, and being responsive to team needs. Being organized with excellent follow through is essential to oversee client needs. We are looking for those with a "can-do" attitude and who are persistent in meeting our SSVF program goals to support a valuable group - our Veterans. WHAT YOU WILL BRING ESSENTIAL
Bachelor's Degree in Social Work or related Human Services field; Master's Degree preferred (equivalent work experience may be considered), and at least 1-years' experience working in the "field." Skilled in formulating creative strategies. Exceptional communication and problem solving skills; adept at taking initiative to seek out resources and solutions. Ability to instill confidence and inspire commitment to the goals of the organization. Excellent relationship building skills, strong organization, and follow up skills. Strong interpersonal skills, including ability to communicate effectively and professionally with staff and community partners and to build productive relationships. High degree of initiative and ability to motivate and engage the community. Must be able to successfully manage multiple priorities. Demonstrated ability to work with high degree of autonomy and accountability and to be a team player. Skilled in Microsoft Office (Excel, Word, Outlook, and PowerPoint) is essential. DESIRED:
Veteran status a plus Previous experience in working with homeless individuals and families, particularly chronic a plus. At least one year of working in a program serving veterans is preferred. Housing First/Harm Reduction approach experience. WHAT WE WILL BRING
