The QI/QA Specialist for practice transformation will have primary responsibility for directing and managing clinical improvement initiatives of Nhan Hoa, ensuring regulatory compliance and promoting improved health care outcomes. The incumbent will work closely with the clinic's CMO and clinical team to establish a vision and direction for the quality improvement and quality assurance program within the framework of current regulations, market initiatives, existing program work plans and the network's strategic plan. The use of health information technology (electronic health records, patient portal, data repository, etc) and resulting data will be a major component of the QI/QA program.Responsibilities:
Designs and oversees implementation of clinical quality improvement and quality assurance programs with the guidance of the CMO. Supports clinical team in developing and implementing the necessary teams, tools and team changes to improve patient outcomes and develop innovative approaches to primary care delivery reform. Participate in critical analysis of individual center and network aggregate results of existing clinical outcome measures, while working closely with Nhan Hoa's clinical staff and informatics resource personnel to develop additional performance measures. Routinely review provider charts for proper measures. Make sure provider and clinic follow current up-to-date guidelines. Works with Risk Manager to ensure the clinic is up to date on current credentialing Review and prepare charts for routine internal auditing. Assist with UDS and HRSA Quality measure Review QI tools and surveys and provide technical assistance to staff Review and analyze patient satisfaction survey. Coordinating tracking and reporting of clinical outcomes, and follow-up of corrective action plans Assist in development, monitoring, and presenting internal quality measures Prepare QI reports and organize monthly QI/QA meeting Works closely with Nhan Hoa staff to ensure that all clinical setup and workflow development is aligned with QI/QA program requirements, goals and objectives. Participates in on-going development of clinical data repository and dashboard reporting. Participates in monthly QI/QA meetings and perform requested follow-up duties. Assists with annual UDS and OSHPD reports. Assist with annual FTCA application related to QI. Assist planning and development of PCMH activities, including working with PCMH team to develop content of PCMH Learning Sessions and webinars, and managing all aspects of these and other PCMH learning activities. Provides oversight, direction, and support for quality improvement and care management staff in their efforts to support practice transformation with PCMH practices. Provides expert advice to PCMH program leaders on the development, communication and implementation of key issues related to PCMH, including: Clear vision and strategy on how practices will successfully transform to PCMH model Expected timeline for practice participants to implement agreed upon strategy Develops and supports positive and engaged team culture. Identifies and manages staffing for additional program needs as needed. Conducts ongoing staff performance evaluations. Other activities and policies that support effective and measurable practice transformation. Produces summary program reports summarizing key goals, accomplishments, and outcomes. Stays abreast of new developments and industry trends related to PCMH, clinical quality improvement, and other assigned program(s). Works with other relevant provider organizations and statewide groups, as applicable, to coordinate and align quality improvement efforts. Performs other duties and responsibilities as requestedMinimum Qualifications:
Experience working in healthcare strongly preferred Bachelor's degree in health or related field, licensed health care professional preferred. In-depth understanding and minimum 3 years' direct experience in using clinical quality improvement models and methods (e.g., Planned care Model, PCMH, rapid cycle improvement, quality measurement and reporting, clinical microsystems improvement). Experience in program design, implementation and evaluation. Extensive knowledge and experience with industry-leading quality improvement methods and tools. 5. Excellent program management skills with demonstrated experience in managing wide range of clinical and/or administrative programs with excellent organizational skills. Ability to work both independently and to collaborate with teams of individuals in diverse settings, with solution-oriented approach. Ability to deal effectively with a variety of people and work in a team environment. Knowledge of computer software, including Microsoft Office. Excellent written and verbal communication skills. Strong organizational skills with the ability to multi-task. Data Analytics experience a plus Ability to maintain detailed and confidential recordsPay:
