This role focuses on quality improvement, patient safety, and regulatory readiness in alignment with CMS Conditions of Participation (CoPs) and hospice best practices. The QA Nurse collaborates with the interdisciplinary team (IDT) to ensure patient-centered, compliant, and ethical hospice care. Key Responsibilities Conduct regular audits of hospice medical records to ensure compliance with CMS Hospice Conditions of Participation, state regulations, and accreditation standards Review clinical documentation for accuracy, completeness, timeliness, and consistency with hospice care plans Monitor compliance with hospice eligibility, recertification, and face-to-face encounter documentation Track quality indicators, including pain and symptom management, care coordination, and patient/family satisfaction Support performance improvement projects related to hospice outcomes and patient safety Provide education, training and guidance to hospice staff on documentation standards, regulatory requirements, and quality best practices Collaborate with clinical leadership and IDT members to identify trends and implement corrective actions Maintain strict confidentiality and compliance with HIPAA and hospice ethical standards Qualifications Current and active Licensed Vocational Nurse (LVN) license in good standing Graduation from an accredited vocational nursing program Minimum of [1-3] years of hospice or end-of-life care experience preferred Strong knowledge of hospice regulations, CMS CoPs, and interdisciplinary care processes Excellent attention to detail, analytical skills, and ability to work independently Proficiency with electronic medical records (EMR/EHR) and
