Job Description

  • The Hospital DME Liaison Responsibilities Include:
  • + Acting as the main point of contact for hospital staff who are discharging patients home with DME (Durable Medical Equipment) and home oxygen.
+ Communicating with Care Coordination and hospital staff to facilitate the timely delivery of DME to patients prior to discharge within the Cerner, HC+ and other Electronic Medical Record Platforms. + Communicating with outside DME companies (as needed) to coordinate the necessary paperwork and delivery of DME. + Collaborating with Customer Service to facilitate timely delivery of DME. This process is meant to reduce readmissions and promote a seamless and timely discharge of patients. + Inventory management of hospital DME closet.

  • Experience:
  • + Required 1 year Health Care Environment or 2 years Customer Service or 6 months Cerner and HC+ Associates Degree in lieu of experience
