Schedule :
Monday - Friday, 9:00 AM-5:30 PM onsite at AdventHealth Zephyrhills and in local area while performing Logistics Courier duties PEA will provide vehicle for use of logistic courier duties only Purpose of Job The Logistics Transition Specialist plays a vital role in obtaining the goals of the vision/mission of PEA. To accomplish these goals, the Transition Specialist must exhibit empathy, innovation, collaboration, teamwork, passion, dedication to personal development, and a strong commitment to "know the patient". The Logistics Transition Specialist is responsible for encouraging patients to play an active role in their patient care experience. These efforts occur during and after the patients' hospital stay. The Logistics Transition Specialist will ensure the patient is provided services, educated on their discharge requirements (medications, patient follow-up care, nutrition, transportation, assistance programs, etc.), and equipped with the resources necessary to be successful in his/her next stage of care. Logistics Transition Specialist is responsible for delivering medications, nutrition, product kits and other patient care related items in a secure manner as needed within the hospital, to the patient's home, or another location for hand off. Position responsibilities of the Logistics Transition Specialist require access to Protected Health Information (PHI). Patient Engagement Advisors' team members are expected to remain compliant with all confidentiality and security policies/procedures related to use, access, and disclosure of PHI. Essential Responsibilities This list of responsibilities is meant to be representative, not exhaustive. The individual in this position may perform additional or related duties as assigned. Patient Engagement / Care Coordination (Face-to-Face and Telephonic Patient Goals) Secure delivery of medications, nutrition product kits and other patient care related items as needed within the hospital to the patient's home or another hospital location for hand off. Offers patient PEA services based on individual patient needs (scheduled appointments, transportation, nutrition, medications, products). Collects and documents social determinants of health information for every patient to whom services are offered (to better understand patient needs and provide optimal services). Utilizes Patient Navigation System (PNS) and other applications appropriately to document completed activities and fulfill patient needs. Acknowledges/understands all PNS patient indicators (high risk, payer type, etc.) and coordinates action steps for improved outcomes.. Meets KPI "Scheduled" goals and ensures follow up appointments are documented in PNS. Collects payments; complies with payment collection related policies and procedures. Utilizes PEA software and assistance programs (coupons, vouchers, discount cards) to provide optimal savings/value for patient. Takes action to facilitate available Generosity services including leveraging partnerships with community resources. Ensures all patients are offered program services; utilizes missed patient reporting to provide services via phone call; provides exceptional care to every patient. Actively participates in hospital-specific rounds/huddles to obtain critical information and contribute as a collaborative member of the clinical care team/hospitalists (ex-communicates available medication programs, unrealized referral opportunities, etc.). Executes daily workflow procedures to ensure KPI thresholds are met/exceeded; Tracks, Engages, Permission, and Serves expected number of patients. Provides pick-up and delivery of prescriptions and medical supplies in an accurate and timely manner. Ensures proper handling and storage of all medications and supplies. Obtains signature of person receiving the delivery. Meets productivity goals, as outlined by site leadership. Submits "Wow Stories" based on PEA Leadership's requested intervals, to Supervisor describing exceptional patient care experience provided. Pharmacy Operations / Logistics Courier Works with prescribing clinicians and pharmacists to ensure affordability and successful delivery of medications per Standard Operating Procedures. Continuing Education & Compliance Demonstrates compliance with PEA and hospital-specific policies and procedures. Maintains working knowledge of specified equipment, services, and supplies available to transitioning patients. Completes ongoing health system and PEA-required training. Maintains compliance with all PEA and health system security and privacy (physical and electronic) policies in all work settings (onsite, remote, virtual). Policies include and are not limited to: phishing attacks / emails, social engineering, password protections / management practices, etc. Qualifications and Requirements Education and Experience High School Education or equivalent required Must be 18 years old or older 6 months of pharmacy technician or 1 year of medical assistant or other healthcare / medical experience required Retail pharmacy experience strongly preferred Other related hospital experience will be considered Associates and/or Bachelor's degree will be considered in absence of healthcare experience Valid driver's license in the state of employment with a clean driving record (driving may or may not be required; any driving requirements may or may not apply) Knowledge, Skills and Abilities Must have and maintain a clean driving record per company policy (driving may or may not be required; any driving requirements may or may not apply). Ability to work a flexible and/or rotating schedule, including, but not limited to, nights, weekends, and holidays. Team members may be required to work during inclement weather (as directed by PEA and facility guidelines) Excellent interpersonal skills; ability to connect with patients/families and multidisciplinary providers in order to build an environment of trust and collaboration Excellent communication skills allowing for optimal patient understanding and engagement Ability to manage multiple, complex patients and address diverse needs Participates in a culture that embraces creativity, productivity, communication, professionalism, and learning Ability to work in a fast paced, rapidly changing environment Creative problem-solving abilities; sound judgement and decision-making skills Consistently maintains a high-level of professionalism; positively reflects the PEA brand/organization, at all times, internally among our team and externally to patients and customers Computer Skills Intermediate computer skills required Web-based tools (including, but not limited to, point of sale, pharmacy information systems, and clinical/EMR systems) Ability to utilize / navigate applications on an iPad Salary range : $20.50-$22.00
