Job Description

Requirements:
Master's Degree from an accredited university in the area of social work or counseling. 3 or more years of relevant experience working with children / adolescents with mental health diagnoses; child welfare experience is a plus.
LCSW / LMHC / LMFT
licensure is required. Driver's License Must be in good standing Excellent written and verbal communication skills Strong organizational skills Proficient in computer programs and software
Salary:
$44,000 to $53,000 / annually / Salary, exempt position to include some evening and weekend hours.
Hours:
Monday to Friday 8:00am to 5:00pm
Job Description:
The Clinical Coordinator is responsible for providing clinical assessment, coordinating care and supporting treatment foster parents to ensure placement stability. Functions as a member of the client's treatment team as outlined in the policy and procedures manual. Recognizes and reports signs and symptoms of abuse and neglect. Progress notes contain clinically appropriate data submitted in a timely manner. Demonstrates knowledge and understanding of client needs based on the age and functioning level of the client. Consistently includes the parent/guardian in client care. Ensures all treatment team members are educated and instructed on individual needs that are pertinent to clients' stay. Demonstrates knowledge and understanding of the client's needs based upon the age and functioning level of the client. Provides education to client and parent/guardian regarding special needs of the client according to the client's abilities. Conducts initial home visit with each child within 72 hours of placement. Completes intake assessment to include treatment recommendations and safety planning at initial home visit. Conducts clinically appropriate number of home visits to each treatment home. Conducts clinically appropriate number of family support group sessions. Provides crisis-call response for assigned treatment foster parents Models appropriate and professional behavior (i.e., dress, habits, personality, tact, etc.). Relates with clients, parents, peers and the general public in a respectful and professional manner in person and phone interaction. Performs other duties that may be assigned.
Physical Demands:
The employee will be required to talk, hear and see; ability to manipulate objects with hands/fingers. Medical Benefits, 401K matching and PTO offered.
