Job Description

Under general supervision of the Health & Human Services (HHS) Department, Health Services Division Clinical Services Manager, provides transportation to patients living within the Community to and from health care-related appointments within and outside the Community boundaries, while ensuring patient safety and well-being. This job class is treated as FLSA Non-Exempt.
Essential Functions:
Essential functions may vary among positions and may include the following tasks, knowledge, abilities, skills and other characteristics. This list of tasks is illustrative only and is not intended to be a comprehensive listing of tasks performed.1. Provides transportation for Community members and their families living within the boundaries of SRPMIC to health care-related appointments within and outside of the Community. Drives several types of vehicles including handicapped vans, mini vans and various sized larger vans up to a 15-passenger van. 2. Monitors safety of all passengers throughout transports. This includes correctly securing wheelchairs within the vans and the passengers in wheelchairs, assisting passengers with special needs and assistive devices, securing all children correctly within the vehicle, ensuring that all passengers are using their seatbelts and ensuring that conduct among passengers is appropriate (no smoking, use of inappropriate language, no use of drugs or alcohol, etc.). 3. Monitors well-being/health status of passengers throughout transport. This includes monitoring the status of dialysis patients for uncontrolled bleeds, slips from consciousness or any other overt sign of distress. Drivers are responsible for quickly assessing patient status and determining need to call emergency "911". 4. Assists patients with special needs at pick up and drop off locations to ensure transition to other qualified staff. This includes assisting those with visual impairments or other limiting conditions with transportation to their appointments and back to their homes. May also include assisting with communication between patients and health provider staff at appointment sites. 5. Fills in for the Dispatcher when asked which includes being responsible for dispatch duties, taking transport requests, coordinating transports throughout the day via two-way radio or cell phone, making confirmation calls to all passengers to be transported the following day, developing daily transport schedules, etc. 6. Maintains vehicle cleanliness and notifies the Dispatcher when vehicle repairs are necessary. 7. Maintains data on all transports. 8. Attends workshops, trainings and in-services to remain current in required competencies (First Aid, CPR, universal precautions, passenger safety, etc.) 9. Performs other job-related duties as assigned by the Clinical Services Manager.
Knowledge, Skills, Abilities and Other Attributes:
Knowledge of history, culture, laws, rules, customs and traditions of the Salt River Pima-Maricopa Indian Community. Knowledge of laws, rules and regulations governing the operation of motor vehicles in the State of Arizona. Knowledge of how to drive various sized transport vehicles. Knowledge of handicapped vehicle lift operation. Knowledge of all passenger safety requirements (securing wheelchairs, securing passengers in wheelchairs, correct installation of child safety seats and how to secure children in safety seats, etc). Knowledge of addresses and services locations throughout the Community and in the surrounding area. Knowledge of methods and techniques for First Aid, CPR and emergency referral procedures in the Community. Knowledge of health services and public health resources available to the Community. Skill following verbal and written instructions, policies and procedures. Skill providing emotional support and advice. Skill safely operating a motor vehicle. Skill performing emergency medical procedures including First Aid and CPR. Skill establishing and maintaining effective relationships with clients, staff, representatives of resource agencies and all health providers. Ability to recognize an emergency health situation and when to call emergency "911". Ability to provide First Aid and CPR. Ability to establish rapport with passengers and their families. Ability to be reliable and flexible. Ability to work a two-way radio. Ability to perform independently. Ability to drive safely. Ability to lift and carry 50 lbs.
Education :
Graduation from High School or a GED equivalent. For enrolled Community Members without a GED, must obtain a GED. Requires participation in the HR GED Program and successful completion of the Arizona state certified exam in order to continue employment with SRPMIC.
Experience:
Must maintain a current certification in First Aid, (including universal precautions/bloodborne pathogen precautions) and CPR. Must be able and willing to take basic Community Health Representative (3 weeks) training course after one year of service. Familiarity with the Salt River Pima-Maricopa Indian Community required. Demonstrated safe experience driving 15 passenger transport vehicles required. Experience working with elderly and disabled people, especially handling their transport, preferred. Some academic training in a health-related field preferred.
Equivalency:
Any equivalent combination of education and/or experience that would allow the candidate to satisfactorily perform the duties of this position, will be considered.
Underfill Eligibility:
An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill. May be required to work beyond normal work hours including nights, weekends and holidays. Must possess and maintain a valid Arizona Driver's License Employees in, and applicants applying for, jobs providing direct services to children are subject to the "Community Code of Ordinances", Chapter 11, "Minors", Article X. "Investigation of Persons Working With Children". New hires in clinical roles must present valid BLS certification (Healthcare Provider BLS for clinicians) prior to the first day of patient care. Ensures all patient information is kept confidential and complies with Health Insurance Portability and Accountability Act (HIPAA) regulations and

policies and procedures. Prior to hire as an employee, applicants will be subject to drug and alcohol testing. Will be required to pass a pre-employment background/fingerprint check.
