Community Life Assistant/Driver:

Participates in the scheduling of activities and programs within the community, communicates those with residents, encourages participation and participates in assigned programming. Conducts assigned programs including 1 to 1 visits. Assists with field trips and outings. Transports residents to and from planned activities within or external to community. Assists with documentation- daily attendance records and 1 to 1 records. Meets with outside local community groups to plan joint projects which benefit community residents as Assures supplies are available for various activities/programs. Gives input to residents care planning- assessments, care plan goals and approaches and progress
