Job Description

As a travel RN specializing in Case Management, you will develop and coordinate care plans for patients, ensuring their healthcare journeys are efficient, safe, and patient-centered. This assignment grants you hands-on experience with new workflows and technology, as well as exposure to interdisciplinary teams—equipping you with unique perspectives and skills valued in the nursing profession.
What You'll Do:
Assess patients' needs, coordinate comprehensive care plans, and manage discharge planning. Collaborate closely with physicians, families, and ancillary staff to guide patient care. Facilitate communication among the care team to ensure continuity and quality outcomes. Advocate for patients and families, ensuring that each case navigates the system efficiently. Document case notes accurately and maintain compliance with healthcare regulations.
What We're Looking For:
Valid and unencumbered RN license. Prior experience in case management (recent and relevant). Strong assessment, critical thinking, and communication skills. Ability to work independently and as a team player. Familiarity with electronic health records is a plus.
