Current, active, and unencumbered Physical Therapist license for the state of assignment (Arizona license required if assigned to AZ). Current BLS/CPR certification from AHA or ARC. Home health experience preferred. Experience providing evaluations, developing and updating plans of care, and documenting patient progress in accordance with physician orders. Experience supervising Physical Therapist Assistants and Home Health Aides preferred. Reliable transportation, valid driver's license, and agency-required liability insurance as required for home visits. Basic computer proficiency including email, electronic documentation, and online training systems. Two to three years of physical therapy experience preferred.

Primary Responsibilities:

Perform patient evaluations, provide therapeutic services per physician orders, supervise and evaluate PTAs and Home Health Aides as assigned, evaluate home environments, and participate in interdisciplinary care coordination.
