Desired Qualifications:
Active and unrestricted RN license Minimum of one year recent experience in Rehab and/or LTC (Long-Term Care) settings Current BLS certification Strong assessment, documentation, and communication skills Adaptability to new protocols and electronic medical records platforms Dedication to compassionate, patient-centered care
Primary Responsibilities:
Deliver comprehensive nursing care to patients in rehabilitation and long-term care environments Collaborate with multidisciplinary teams to develop, implement, and evaluate care plans Administer medications, monitor patient responses, and document changes Educate patients and families about care plans, progress, and ongoing needs Maintain accurate, timely patient records and promote a safe, supportive environment
