  • Contract Overview Join our dynamic healthcare team as a Travel Licensed Practical Nurse (LPN) specializing in Long Term Care (LTC).
In this vital role, you will provide compassionate, high-quality nursing care to residents in various long-term care facilities across different locations. Your expertise will ensure residents receive personalized attention, promote health and safety, and support the overall well-being of individuals with diverse medical needs. This position offers an exciting opportunity to travel and make a meaningful difference in the lives of seniors and individuals requiring long-term support. Responsibilities Deliver comprehensive nursing care to residents in long-term care settings, including assisted living, nursing homes, and memory care facilities. Conduct thorough patient assessments, vital sign monitoring, and physical examinations to evaluate health status. Administer medications, injections, IV infusions, and perform catheterization with precision and adherence to safety protocols. Manage complex cases involving dementia care, Alzheimer's disease, hospice needs, acute pain management, and infectious disease care. Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic or Cerner to document patient information accurately. Collaborate with interdisciplinary teams on discharge planning, case management, and triage to optimize patient outcomes. Follow infection control procedures and aseptic techniques to maintain a safe environment for residents and staff. Skills Proven experience working with individuals with disabilities, developmental delays, or behavioral health needs. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and CPT coding for accurate documentation. Skilled in patient assessment, vital signs measurement, specimen collection/processing, and medication administration. Familiarity with EMR/EHR systems such as Athenahealth or eClinicalWorks; proficiency in medical documentation is preferred. Ability to perform advanced nursing procedures including tube feeding, venipuncture, sterile processing, and spinal taps. Excellent critical thinking skills with experience in acute care environments such as ICU or Level I Trauma Centers; hospital medicine experience is highly valued. Knowledge of infection control standards, HIPAA compliance, and emergency response protocols including airway management and ventilator support.
